If you have successfully completed one of the EPOCRATES/REALCME Clinical Updates and wish to request an official Jefferson CME Certificate for your records, please provide the following information. We will match up your request to the completion data we receive from Epocrates, award your credits, and email you when your certificate is ready to be downloaded. We suggest use of the last four digits of your SS# or a number you will easily remember Our online certificate retrieval system works by using the "Web ID" as a unique identifier. We combine your last name and a 4-digit code to create the Web-ID. ALL your Jefferson CME records will be tied to this unique identifier WEB-ID.
