Group Discount Registration

Group Registration Instructions:

Groups of 2 or more physicians can attend the conference for a discounted rate of $290 per person. To be eligible for this rate:

  1. All members of the group must be registered at the same time.
  2. There are two forms to be completed for Group Registration.
    1. The Group Registration Form, which identifies the organization, contact person, and payment terms; and
    2. the Group Registration Excel Document (linked Excel document), which identifies the individuals who are to be registered, and the dates of attendance.
  3. Payment for the group must accompany the registration documents

Submit the Group Registration Form via email, along with the Group Registration Excel Document to Rachael.Buboltz@jefferson.edu
