Q: MP shows no signs or symptoms of CHF at this visit, and her weight is stable. How should you proceed?

A: In the absence of signs or symptoms of CHF, the AHA/ADA Consensus Statement recommends continued observation of the patient for increasing edema. The dose of the TZD may be reduced if the edema is not tolerated, or a diuretic can be added. Because MP’s blood pressure is not controlled to the recommended level (<130/80 mm Hg),9 you might consider adding hydrochlorothiazide or low-dose furosemide at this juncture, although the effectiveness of diuretics in TZD-related edema may vary.1
