Q: What is the pathogenesis of TZD-associated edema?

A: Multiple factors probably contribute to fluid retention and peripheral edema caused by TZD use, although the reasons are not fully understood. An increase in plasma volume may be a result of a reduction in renal excretion of sodium and an increase in sodium and free-water retention.2 Increased sympathetic nervous system activity,3 activation of vascular endothelial growth factor (VEGF),4 and changes in endothelial permeability5 may all contribute to this phenomenon.
