Q: What is the purpose of adding a TZD, which improves insulin resistance in muscle and adipose tissues, if this patient’s primary defect is insulin secretion, not insulin resistance?
A: As is the case with metformin, TZDs do not work effectively when there is insufficient insulin present. However, when used in combination with insulin, rosiglitazone and pioglitazone further lower fasting glucose levels by 30 mg/dL to 50 mg/dL and HbA1c levels by 0.5 to 1%.9-10 Allowing insulin to work more effectively can also lower insulin requirements. The use of metformin or TZDs should be monitored closely in this type of patient, and if oral therapy does not improve PD’s overall control, it should be discontinued, and insulin should be used alone.
