Q: What effect will PD’s weight have on her insulin requirements?

A: It is important to appreciate the difference in insulin requirements for lean and obese patients with type 2 diabetes mellitus. Insulin resistance and hyperinsulinemia are the classic abnormalities of obese patients with type 2 diabetes. Insulin secretion is often already elevated but is insufficient to overcome insulin resistance. It can be predicted that lean patients with mild or moderate insulin resistance will require considerably less insulin to control their hyperglycemia than will obese patients with high levels of insulin resistance.
