SW’s Primary Care physician (PCP) starts him on hydrochlorthiazide 25 mg daily. In 2 weeks, his blood pressure has decreased to 140/86 mm Hg. His physician adds aspirin (81 mg) daily.
Q: What is the recommended initial therapy for patients with hypertension?
A: The JNC-7 guidelines3 recommend that thiazide-
type diuretics be used as initial therapy for most patients with hypertension, either alone or in combination with one of the other classes with demonstrated benefit. In clinical studies, including the recently published Antihypertensive and Lipid Lowering Treatment to Prevent Heart Attack Trial (ALLHAT),4 diuretics have been virtually unsurpassed in preventing cardiovascular complications of hypertension. Diuretics also have the advantage of being more affordable than other antihypertensive agents.
