Q: What are the expected results if a TZD and statin are used in combination?

A: In one randomized, double blinded trial,6 rosiglitazone alone demonstrated a modest increase in LDL-C (9%), a shift in LDL phenotype from dense to large buoyant subfractions (52% of patients), and an increase in total HDL-C levels (6%), predominantly in HDL2 levels. When atorvastatin was added, there was a further increase in HDL3 (5%), and significant (p<0.0001) reductions in LDL-C (-39%) and triglyceride levels (-27%). Glycemic control achieved with rosiglitazone alone was not adversely affected by add-on atorvastatin. A retrospective chart review of patients with type 2 diabetes treated with pioglitazone in combination with statin therapy showed similar beneficial effects on total HDL-C and triglyceride levels.8
